Injury, Liability, Photo, and Social Media Release Waiver

I do hereby agree to participate in the Martial Arts Programs at “Absolute Brazilian Jiu-Jitsu Inc” (referred to as “Company”) located at 80 Bishop Drive, Fredericton, NB. The responsible party must read the entire contract before signing.

I agree to the following terms and conditions:

1. Assumption of Risk:

I am aware that martial arts involves physical contact, including grappling, joint manipulation, throws, and submission techniques, which may result in potential risks of injury, including but not limited to bruises, strains, sprains, fractures, and more severe injuries.

2. Release and Waiver of Liability:

I recognize the risks of injury that are common to any martial arts program that my child or I participate in and I hereby release, discharge, and hold harmless the Company, its instructors, staff, affiliates, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of my participation in any martial arts programs or activities, including any injuries sustained during training, sparring, or competition with the company.

3. Medical Responsibility:

I acknowledge that it is my sole responsibility to assess my child or my own physical fitness and health to engage in martial art activities. I agree to seek medical advice from a qualified physician before participating if I have any concerns regarding my child’s or my physical capability to engage safely in these activities.

4. Photography, Video, and Social Media:

I grant the Company the right to capture and use photographs, videos, or recordings of my child or myself during classes, events, or tournaments for promotional or educational purposes on the academy's website, social media platforms, or other marketing materials. I understand I can revoke this permission at any time by communicating such in writing to the company via email at info@absolutebjj.ca

5. Confidentiality and Privacy:

I understand that while the Company maintains confidentiality standards, participation in group classes or events might involve inadvertent disclosure of personal information or images. I waive any claim or right to privacy concerning such information captured during activities.

6. Agreement to Terms:

I have carefully read and understand this Waiver and Release of Liability for activities within the Company and agree to abide by its terms and conditions. I acknowledge that this agreement is legally binding upon myself, my heirs, executors, and assigns.

I affirm that I have had the opportunity to seek legal counsel regarding this Waiver and Release of Liability and am voluntarily accepting its terms.

Signing below indicates that you have read the Waiver and Release guidelines, terms and conditions, and regulations on this contract and fully understand the contents. This contract will remain on file unless the terms and conditions change. At that time a new contract will be executed.
